This policy covers information collected through this website. It does not cover protected health information (PHI) handled under our service agreements with partner communities. That information is governed by those agreements, by HIPAA and by any applicable business associate agreement — not by this page.

Information We Collect

  • Information you give us. When you submit the contact or careers form we collect what you type into it — for example your name, email address, phone number, organization, care setting, approximate census, any message you write, and a resume if you attach one.
  • Information collected automatically. Our hosting provider records standard server request data such as IP address, browser user agent, referring page and timestamp. This is used for security and reliability.

The forms on this site are for business inquiries and employment applications only. Do not include resident names, medical details or any other protected health information in a web form. Partner communities should use the dedicated triage line for clinical matters. If a resident needs immediate medical attention, call 911.

How We Use Information
